Your engagement with PiC

The key to efficient and effective referral management is communication. Initial referral can be a difficult time for the patient, referrer and commissioner alike. We passionately believe the whole process of referral, assessment and admission should be underpinned by clear communication and quality of response that enables you to confidently refer a patient to our services.

How to make a referral

Referrals can be made to our central referral line – 0800 218 2398 – or by email to referrals@partnershipsincare.co.uk. Alternatively if you know which hospital you wish to liaise with please see our hospitals section from page 30 for contact details and ask to speak to the referrals manager. The PiC referrals manager will discuss with you the needs of your patient and agree the most suitable hospital.

Our normal protocol will be to assess every patient in person, though in some cases, an assessment decision may be made upon discussion with the referrer using the available clinical records. At this point your ‘key contact’ will work with you to ensure a smooth transition from first contact to admission.

Your key contact will ensure you are fully supported through the assessment

The referral and the assessment

As soon as we receive your referral details, we will pass them to the appropriate clinical team for review. Your nominated key contact will contact you with an initial indication of suitability for assessment within 24 hours. We will then manage the entire referral process, in collaboration with you.

Assessment of the patient will normally take place within 48 hours. We will contact you the next working day after the assessment with a placement recommendation. A formal report will follow within 5 working days.

In an emergency we can assess and admit a patient within hours of referral.

With 23 hospitals, it will be a rare event when we cannot meet the needs of your patient.
