Admitting the patient


Maintaining our commitment to communication, we will make contact with the commissioner/referrer to report on the transfer process, admission date and answer any immediate questions.

During the patient’s stay


At PiC, we are committed to minimise the length of inpatient stay. Our clinical teams will seek to maximise every opportunity to speed the patient’s progress along the agreed treatment pathway. The clinical team will continually review treatment plans, developing challenging timelines that are agreed with the patient and commissioner/referrer. This will ensure the treatment experience is both rewarding and progressive.


Through close collaboration with other PiC regions we will suggest repatriation within our own network of services where this may be beneficial for the patient.


Throughout the patient’s stay, we will continually ask ourselves:


• Does the patient continue to require this level of security? • Are we doing everything we said we would? • Is everyone included that should be (e.g. is the local care co- ordinator involved)?


• Does the patient agree with and understand their care pathway? • Is the shared pathway clear to all stakeholders?


Internal challenge encouraged by our clinical teams, ensures that the review of shared decisions reflects a process of continual dynamic decision-making, integral to ward rounds, CPA and other clinical reviews.


Cases of particular complexity may also be subject to cross-team peer-review – an opportunity to pool and share the breadth of skills across clinical teams in the development of treatment plans and care pathways.
