The Medicines Management pharmacists identifi ed that a social care medication support package would facilitate more holistic prescribing decisions and so they approached the Prescribing Action Group, a multi- disciplinary group working across health and social care within the Motherwell locality, who agreed to test the concept. After discussions with the GP, a fi nal plan of changes for individual patients/service users was agreed, discussed with the patient and their family carer, and implemented following agreement.
M&D GREEN DISPENSING CHEMIST PARTNERSHIP – BELLFIELD PHARMACY, KILMARNOCK
The Bellfi eld pharmacy team has established a very effective working partnership with the district nursing teams located in East Ayrshire, primarily in and around the Kilmarnock area. Over the past couple of years, each member of the team has built up an excellent working relationship with community nurses and now three separate teams of district nurses direct all of their prescriptions and patient care needs towards the pharmacy; thereby maximising standards of patient care.
ROWLANDS PHARMACY, MINTLAW, ABERDEENSHIRE
The Mintlaw area is a rural community with a single GP practice and a population of approximately 10,000. As such, the pharmacy has a strong working partnership with the GP practice and actively seeks out any opportunities to work together on patient care initiatives. The pharmacy actively promotes the minor ailment service, which not only gives patients quick access to effective medication and useful advice on self-limiting conditions, but also frees up appointments at the GP practice.
LLOYDSPHARMACY CUMNOCK HEALTH CENTRE, CUMNOCK
Pharmacist Laura McKinnell is involved in a project involving a ward of a local hospital, which is primarily GP-led, but which is run by GPs based at different practices. As such, there was no set process in place for handling discharge prescriptions, which caused confusion for the nursing staff. Laura has taken on a clinical role, visiting patients on the ward to discuss their understanding of and compliance with medication. She also fl ags up issues to the relevant healthcare professional.
