HOSPITAL PHARMACY TEAM OF THE YEAR

The Royal Infi rmary of Edinburgh (RIE) is the largest acute teaching hospital in NHS Lothian with 31,000 patient admissions annually and an average of 125 discharges and admissions per day. Against a challenging background of increasing fl ow and patient acuity, traditional models of clinical pharmacy service delivery no longer met the requirements of patients in acute care. A core clinical pharmacist/technician team was established, which included the expertise of the most experienced clinical pharmacists and a clinical pharmacy technician from the group to design, test and embed a more risk- focused rather than ward-focused approach to service delivery.
PHARMACY DEPARTMENT, BORDERS GENERAL HOSPITAL, MELROSE
The Borders team identifi ed that, by clinically prioritising workload on a daily basis, they were able to review their staffi ng requirements on a daily and weekly basis. As a result, the team adopted a new rota and changed their work pattern with the aim of structuring and concentrating work within set time frames for the CIVAS (Central Intravenous Additive Service) part of their service. This resulted in less downtime in between work items while waiting for prescriptions to arrive; freeing up staff time to enable them to help in other areas of pharmacy service provision.
COMBINED ASSESSMENT UNIT PHARMACY TEAM, UNIVERSITY HOSPITAL CROSSHOUSE AND UNIVERSITY HOSPITAL, AYR
The Combined Assessment Unit (CAU) at the University Hospital Crosshouse (UHC) opened in April 2016 and CAU at UHA followed in 2017. Both employ the Hospital Electronic Prescribing and Medicines Administration (HEPMA) System. It had been recognised that focusing on the ‘front door’ would yield the most effi cient way of providing pharmaceutical care to the majority of patients admitted to hospital and this resulted in a new team being created to serve the CAU cohort with a focus on improved patient care and improved fl ow. Funding was obtained for a designated pharmacy team specialising in unscheduled care.
