Hello again, yes, we are doing breast implants on a day surgery basis using sedation without the need of general anaesthesia. Actually we are also doing breast uplifts and reductions under the same.
It requires a first medical assessment just to make sure that the patient is suitable for the procedure; most patients are suitable, but I am very cautious regarding some pre-existing conditions such as heart problems, asthma, diabetes or long term use of some drugs such as steroids. In these cases hospitalisation can be required in order to prevent complications. The procedure is usually carried out under deep sedation in which the patient is put to sleep but does not require intubation or life support during the procedure. We also use local anesthetics in combination so no pain should be felt even if the patient wakes up during the intervention. Sedation is strong enough to put the patient either in a deep sleep or to the twilight point in which she will take no notice and will probably won’t remember anything afterwards.
The procedure for breast implants takes about an hour and a half and when finished, the patient is woken up, dressed in bandages and sent home. Because she may be still under the influence of the anesthetic, it is always mandatory to have a “responsible adult” to stay with the patient overnight. The patient must come back to the clinic to be seen again the day after and then once a week for the following 3 weeks. Stitches are given under the skin and only the tips of them need to be removed after 10 to 14 days.
Question: Dear Dr. Poole I am a woman of 55 and although my face looks alright to me, I find that my neck has given way a lot. I don’t think I want a full face lift, only the neck. Is that possible? And how does it work?
Hello, I have said in the past that there is no such thing as a “mini” or “partial” lift, that mini ops lead to mini results and that although sometimes our worry is about a certain area, the rest is almost the same. Having said that, it is also obvious that in some cases a part of the face is more noticeable than the others or simply the patient doesn’t want to have a complete job as they are happy with the rest. For these cases the approach is to deal with the affected area only but making sure that the result will be natural. To give you an idea, just pull your neck backwards to the desired position. You will notice that the skin will create a fold or a lump in front of the ear that will look really awful. For that the procedure is similar to complete the face lift, but only adjusting the skin on this area. In other words, it is a partial face lift but with some skin adjustments.
Question: Dear Dr. Poole. I have been using botox on my forehead and frown for the last two years. Although I am happy with the results, I find that after 3-4 months the wrinkles gradually come back and that I have to re-do it every 6 months. I have been told that a ‘top lift’ can eventually get rid of these unwanted wrinkles in one go. Can you please help me understand more about this? Many thanks.
The forehead lift is a simple procedure carried out with local anaesthesia on a day surgery basis. It only takes about one hour and it consists of two incisions of about two inches long placed behind the hair line on both sides of the forehead, above the temples. Skin and muscles are lifted from there to the eyebrows. Internal sutures are given to support and anchor the muscles to a higher position and then the scalp is pulled backwards about an inch and sutured to its new position. The stitches can be removed after seven days and the result may be considered stable for a number of years. The advantages over botox are that it doesn’t stop the person’s expression as the muscles can still move as normal, and that there will be no need for after care or re-injection for a very long period of time. Also, the hair is never cut and extra care is taken to ensure that the incisions will always be covered by the patients’ hair. Here is a link that shows how the whole procedure is done:
