MANAGEMENT OF


LONG-TERM CONDITIONS ENTRY CAN BE BY NOMINATION OR BY PERSONAL APPLICATION


The Chronic Medication Service is one of the core elements of the community pharmacy contract in Scotland. Pharmacists are now tasked with care planning and sometimes serial dispensing for patients, putting them at the front of patient care. For the first time patients with long-term chronic conditions are now primarily managed by pharmacists on a national scale.


We want to recognise a pharmacist or pharmacy team which has embraced the Chronic Medication Service, and which will have delivered continuously high- quality care, with evidence of improved outcomes for individuals. Are you or do you know someone who is excelling in delivering CMS?


GENERAL AWARDS CRITERIA:


• Enthusiasm and commitment to the delivery of high-quality care with evidence of improved outcomes for individuals.


• An ability to overcome challenges in pursuit of goals • Evidence of how performance has been measured and benchmarked, in addition to evaluation of project’s success


JUDGES WILL BE LOOKING FOR:


• Demonstration of excellent practice when delivering CMS to patients • Evidence of collaborative working with patients to help facilitate self management of their condition


• Examples of identified care issues which have been addressed, improving patient outcomes


WHO CAN ENTER?


This category is open to all pharmacists working in either independent or chain pharmacies. Previous Management of Long-Term Conditions Award entrants, including winners, are also welcome to submit an entry. All entries will be judged by an independent panel of experts.

The Pharmacist of the Year Award recognises a pharmacist who demonstrates leadership and exemplifies the evolution of the pharmacy profession toward an expanded role in health care.


The winner will demonstrate a significant contribution to the pharmacy industry overall, resulting in meaningful improvements in the quality of patient care, delivery models, and pharmacy’s role on the entire healthcare team.


Applications are invited from both the independent and chain sectors. JUDGES WILL BE LOOKING FOR:


• Evidence of the measurable impact that has been made on community pharmacy in the past year


• Detail of action taken to promote or improve pharmacy standards • Evidence of how individual has stood out from the crowd • Evidence of how individual has encouraged or inspired others to progress


WHO CAN ENTER?


This category is open to all pharmacists working in either independent or chain pharmacies (including locum pharmacists). Previous Pharmacist of the Year Award entrants, including winners, are also welcome to submit an entry. All entries will be judged by an independent panel of experts.
