The aims of treatment are to eliminate the bacteria, to ensure any foreign bodies i.e. grit or thorns are removed, to restore a normal pH to the joint, to avoid future joint degeneration and to prevent the spread of infection throughout the body. The principles of treatment are a high dose of antibiotics and lavage of the joint capsule with a high volume of fluid. The lavage of the joint is the key part of treatment. Not only does it remove any foreign material and bacteria, and restores the pH to normal, it also gives the antibiotics the best chance at working. Lavage of joints is most easily carried out via arthroscopy (keyhole surgery). This allows the surgeon to visualise what is happening inside the joint capsule and facilitates a high throughput of sterile saline. An alternative is to flush the joint using needles and drains however this does not have as good a prognosis as arthroscopy, due to the lack of visualisation.
The outcome for a septic joint is dependant on prompt recognition, aggressive therapy, good nursing and monitoring post operatively. Horses have a much better prognosis following a septic joint now than they did 10-15 years ago.
