Index (BMI) is 25 (20-32) and operative time is 130min (86-205). Blood loss is not significant (we report four bleedings of 70; 100; 120 & 150cc). Only the anterior mesh is placed in five


patients, lowering the operative time (96 min). We report one complication: a 10mm vaginal wound that has been sutured without standard laparoscopic or open conversion. In this last case, the mesh was nevertheless placed and we did not report any fistulae after the 13 month follow-up. There was no need to place any additional trocar, we did not report any standard laparoscopic or open conversion. To this day, there has been no recurrence with a mean follow-up of 12

months (1-23). Like many authors, we think this technique appears to offer excellent and safe results. All LESS procedures are safe because it is always very easy and quick to place an additional trocar if needed, especially during the learning curve. The operative time is estimated to be 20% longer than with standard laparoscopy in our personal experience but the cosmetic results are better. In all series operative time is equivalent in LESS procedures if there is no suturing steps. The learning curve is estimated to be between 10 and 20 procedures for a laparoscopically trained surgeon depending on the procedure. We recommend to begin with short and easy


procedures. Some training sessions, some tips and tricks may also be useful to know before beginning such an activity. We think, like many authors, that LESS


procedures will develop in the future due to the comparative and safe results. But this technique will also develop due to the cosmetic advantages and the high level of satisfaction in the patients. The effect of pain and recovery on a single access cut is highly probable but still to be scientifically demonstrated. LESS surgery is now routinely performed in our team and we hope many others will join us. ■
