he Gastroenterology and Rheumatology Pharmacy Team at Western General Hospital, NHS Lothian, have redefined the
role of hospital pharmacy in modern patient care. Recognised for their innovation and multidisciplinary leadership, the team stands out as a model of excellence in specialist care.
Described as visionary, dynamic and integral to the successful delivery of novel and complex therapies, the team has pioneered numerous clinical and operational advancements in the past year. From championing biosimilar switching to expanding pharmacist-led clinics, their work has combined cutting-edge practice with a human centred ethos to make a measurable impact across the hospital and wider health board.
One of the team’s most high-profile achievements has been their leadership in the biosimilar switch programme for high-cost biologics such as ustekinumab and tocilizumab. These switches have delivered significant efficiency savings while

maintaining the gold standard of care, an essential achievement during a time of continuing financial strain. Patients were engaged from the outset, provided with clear information and supported via helplines, ensuring a smooth and safe transition with no compromise to outcomes.
At the heart of their success is a bold approach to pharmacist-led service delivery. This includes the development of post-induction review clinics and Virtual Biologic Clinics (VBCs), where pharmacists take an active role in assessing patient responses to treatment. Through clinical examination skills and a collaborative approach with consultants, they help adjust dosages, switch therapies where necessary and optimise patient care. These innovations have reduced treatment waiting times from eight weeks to just three, unlocking vital capacity for consultants to focus on more complex patients.
Beyond direct clinical care, the team has developed treatment pathways for inflammatory
bowel disease and rheumatic conditions, now used throughout the southeast region.
A key element of their impact lies in their interdisciplinary collaboration. Whether it is working closely with consultants to streamline biosimilar implementation or partnering with procurement to ensure efficient medicine supply chains, the team plays a central role in coordinating care. Their work on Homecare services has improved patient access and compliance by allowing delivery of specialist medicines directly to patients, reducing the burden on hospital visits and enhancing quality of life. The team’s influence extends well beyond their own walls. Their work is shared nationally, with team members taking on leadership roles, chairing professional groups and presenting at conferences. Their expertise in managing biologics is widely sought after and they are actively shaping the direction of pharmacy services across NHS Scotland.
