LYNN KING, GLASGOW CITY (SOUTH) HEALTH AND SOCIAL CARE PARTNERSHIP AND NHS GREATER GLASGOW & CLYDE


The main objective of this initiative was to form a partnership between secondary and primary care teams that would allow patients attending hospital with lung cancer to be referred to a GP practice based prescribing support pharmacist (PSP) for clinical medication review within their GP surgery. It was hoped that the initiative would forge a link between Improving the Cancer Journey (ICJ) team, secondary care lung cancer team, secondary care pharmacists, primary care PSPs and the GP practice team. In June 2017, the first of several meetings took place with the lead PSP for the pilot (Lynn King) and the Head of ICJ. During this time, it was agreed that newly-diagnosed patients with lung cancer on four or more medicines and/or with a problem with medicines would be the pilot patient group, and the patients would be registered with a GP in Glasgow South. The development of this pathway has necessitated new and ongoing communication


and inter-professional working between primary and secondary care multidisciplinary teams (pharmacists, nurses and doctors), which has, in turn, improved the quality of care for patients and their carers. The PSP utilises the secondary care pharmacist’s expert knowledge and experience on cancer medicines and the secondary care pharmacist can consult a PSP on issues that are best managed in primary care.
