WINNERS HOSPITAL PHARMACY TEAM OF THE YEAR

The renal pharmacy team from University Hospital Crosshouse (NHS Ayrshire & Arran) – Aileen Dunleavy, Permjit Kaur Singh and Katy McCormack - had identified an opportunity for the delivery of a pharmacist-led clinic for the provision of tolvaptan therapy to patients across NHS Ayrshire & Arran. Tolvaptan is indicated to slow the progression of cyst development and renal insufficiency of autosomal dominant polycystic kidney disease (ADPKD) in adults with CKD stage one to four with evidence of rapidly progressing disease. The pharmacy team identified that they could provide an efficient, patient-centred and convenient service for this patient cohort, ensuring that all safety measures were complied with. Significant planning was applied to ensure the delivery of this service met the objective of being patient- centred, convenient for patients to access, have appropriate monitoring in place and maintain patient safety. Patient information resources


were developed to provide patients with information on clinic format and treatment, together with production of a checklist to ensure both pharmacists were providing a consistent patient experience. A clinical renal pharmacy email box was set up to facilitate the organisation and co-ordination of the taking of blood samples in the community to avoid the need for unnecessary further hospital appointments. A clinic structure was also established for patients to attend for review by a renal pharmacist and receive an ongoing prescription provided by a pharmacist independent prescriber.
