Possible signs and symptoms: • Failure to gain or maintain appropriate weight • Fear of food • Irrational beliefs about the effects of food on the body • Restriction or refusal of food or energy-dense food • Refusal of food due to fear of purging

Possible etiologies: • Diagnosis of bulimia nervosa, binge eating disorder, or eat- ing disorder not otherwise specified (EDNOS)
• Disordered eating pattern • Resolution of prior hypermetabolism without reduction in intake
• Overfeeding of parenteral/enteral nutrition • Calories unaccounted for from IV infusion and/or medications
Possible signs and symptoms: • BMI greater than 25 (adults) • BMI greater than 95th percentile for age and sex (children and adolescents)
• Body fat percentage greater than 32% (women) or greater than 25% (men)
• Weight gain • Intake of foods with high energy density and/or large por- tions of food/beverages
