HOW MANY DIFFERENT TYPES AFFECT YOUR DEPARTMENT? Last month we reviewed some


facets of failure that are fairly common, yet not often acted upon. To review, they are: - Situation: quit and cut your losses - Interactions: trust - Interactions: respect: person, organization, idea, outcome


- Interactions: coordination/ collaboration


- Interactions and mental models: misplaced priorities This month we’ll combine the


above (situation, interactions, mental models) to continue searching for the root cause. Falling short of this endeavor can lead to rework, declining morale and lowering of safety standards.


NOT A SOLO EVENT As we saw last month, failure envelops more than one person, one activity, one instance. Failure is the natural progression of inattention, complacency, preoccupation and (possibly) succumbing to urgency. Here are some of the less thought of facets of failure: • Beliefs inside your head: What are you telling yourself and others? How often have you told yourself


it won’t work, or rationalized why it did not? Do you go into a project saying (either to yourself or to others) “Gee, I don’t know if this will work,” or, “If we pull this off it will be a miracle!” Aren’t you really saying you have a bona fi de reason if it does not

work? If you do not believe it will work, then it will fail. Your failure was part of self-defeating behaviors and a lack of belief in your abilities and the abilities of your team. • Behaviors: that’s how others judge you


How many times have you told someone that you will try to complete it? Try is one of those ambiguous words — the person hearing it believes that you will do it. You are telling them you probably won’t, and you don’t want to discuss or debate it. You have failed before you haven’t even begun. • Interactions: sharing the workload and the brainpower
