A SERVICE TO BE PROUD OF
It’s the envy of most pharmacists outside of Scotland and yet it’s something we do each day without thinking about. I am, of course, talking about the Minor Ailments Service. We shouldn’t forget how revolutionary having a national service of this nature truly is. It empowers community pharmacist to independently treat patients on the NHS and that’s something we should, quite rightly, be proud of.
What makes the Scottish system particularly special is that pharmacists have the opportunity to prescribe from the full range of P and GSL products available on the NHS. This gives us a fantastic choice of treatments and the ability to treat a huge spectrum of ailments. In contrast to this, I’ve worked in England with a local minor ailments scheme that was limited to a dozen treatments for a handful of conditions. These restrictions were frustrating for pharmacists, confusing for patients and ultimately meant the scheme missed its main objective, keeping patients with minor ailments out of GP practices and A&E.
The breadth of our Minor Ailments Service also facilitates the speedy inclusion of new treatment options which are quickly made available to pharmacists as a prescription choice. One example of this is Vamousse, a new generation head lice product which eliminates all lice and eggs on contact with no need to reapply. This relatively new head lice treatment option is now available under eMAS in Scotland, unlike anywhere else in the UK.
In Scotland, being allowed to exercise our clinical and professional judgment with product choice demonstrates not only our competence, but also our ability to take on clinical responsibility. Of course, we have some support with the assistance of local formularies, which, along with other national and international guidelines, can help inform our choices without necessarily constraining us. This is absolutely key to the underlying principle that the pharmacist is deploying their clinical expertise to treat the individual patient. It also means, however, that pharmacists need to keep up to date with the latest treatments, as formularies and guidelines may only be reviewed periodically and can therefore go quickly out of date.
In the future, I believe we can benefi t from an enhanced eMAS scheme based on some of the work currently being piloted around Scotland. This could mean treatments for other common self-limiting conditions, such as urinary tract infections and impetigo, would also be included under eMAS. I hope this will be the case, as it really does demonstrate that Scotland is prepared to develop the role of community pharmacy within the NHS.
HIGH NOON FOR HEAD LICE
Vamousse, head lice treatment, is now available on the Minor Ailment Service. Developed by US-based company TyraTech Inc., Vamousse is a non-greasy, easy- to-apply mousse, which kills 100 per cent of lice and eggs within 15 minutes of contact. It works by removing the protective outer lipid layer of the lice, which leads to their quick dehydration and death.
