TAKING A CMS FOCUS ON NEW DIRECTIONS

Davidsons Chemists are the largest independent multiple chain in Scotland with 32 branches.


Milnathort is a small town in Perthshire in close proximity to neighbouring Kinross, and a hub for surrounding villages. Their pharmacy serves a population of around 6000 patients with signifi cant numbers of elderly patients and young families.


For Davidsons, the advancement of the Chronic Medication Service, and some of the key points in Prescription for Excellence meant that CMS shouldn’t just be about registration fi gures and preparation for serial prescriptions.

They identifi ed the need to develop a focus on pharmaceutical care issues, and, most importantly, quality pharmacist input into patients with long term conditions. In 2014, they appointed Alasdair Shearer as CMS Quality Champion. Alasdair has demonstrated a keen focus on CMS, through his roles on the health board’s CMS implementation group and the PCR user groups.


Since starting in the Milnathort pharmacy in June 2014, Alasdair has increased registrations by 45%, but with a focus on identifying care issues and new medicines interventions, which have seen a 122% and 68% increase respectively.


It is this focus on quality input into patient care that Davidsons Chemist want to promote within the company. Alasdair’s role was created with the purpose to focus on helping both individual pharmacists, and the wider company to focus on providing


personalised, comprehensive and continuous care to our patients through CMS interventions.


He has focused on a number of key areas to do this:


Personal Support: Alasdair has provided one to one support for our pharmacists, being released from his daily duties at Milnathort to provide face to face support for Davidson’s pharmacists. This has allowed him to identify key areas to focus on, and come up with a number of tools that have now been implemented across the company, and beyond, to help pharmacists deliver quality pharmaceutical care. By drawing on excellent examples of care planning and patient interventions, Alasdair is then able to mentor and coach other pharmacists to make quality care interventions, and record, review and even refer using the PCR tools.


Focus on Key Clinical Areas: Very quickly into the role, Alasdair


recognised that pharmacists responded well to focusing on individual conditions, and with training and support, would be confi dent in producing a more involved care plan for patient’s in those groups. In line with this, Superintendent Karen Gordon had previously identifi ed the need for focused clinical training nights. These would be attended by all pharmacists in the group and focus on one set clinical topic every quarter. There would be an educational half to the evening; allowing pharmacists to refresh and increase their knowledge of a chosen subject, and then the second half focuses on linking into CMS, and developing care plans using examples together.


Karen tells us, “The fi rst key area focused on was Asthma. In order to prepare our pharmacists and pharmacy teams, we linked in this disease group with our quarterly pharmacist training nights. The training focused on inhaler technique,
