Hindon Surgery Our Proactive Care Programme

Page 9 TISBURY LEISURE CENTRE

This corner of Wiltshire has the oldest population in the county. Around 30% are over 65 and 14% are over 75 (nationally the government is panicking because 10% of the total population will be 75 or older by 2014).

The NHS wants more planned care for patients most at risk of emergency hospital admission - those who are elderly, have one or more long term conditions and who live alone or in a household with someone in a similar situation. The highest risk is for those who have just had an emergency.

At Hindon Surgery we will be improving our care for this group of patients in two ways:

• Soon after an emergency visit to a hospital or after

discharge a practice nurse or doctor will ring you at home. This is why knowing where you are and having the right telephone number is so important.

• We will be inviting some patients to a half hour afternoon appointment with our new practice nurse, Sally Davies, to discuss a care plan, ways to reduce risk and find out what you want should an emergency occur. We hope to extend this, with the help of community staff, to those who are unable to come to the surgery.

But of course we encourage everyone to engage in the usual preventative care to avoid an emergency in the first place - having vaccinations, regular check-ups for long term conditions, consulting us early when feeling unwell and having a healthy lifestyle and keeping warm.
