Community Aged Care Packages (CACPs)


Community Aged Care Packages (CACPs) are individually planned and co-ordinated packages of care to help older Australians remain living in their own homes. Funded by the Australian government, CACPs provide for the complex care needs of older people who have been assessed by the ACAT team as requiring low-level (hostel) care at home. The types of services that may be provided as part of a package include: personal care, social support, transport to appointments, home help, meal preparation and gardening. To be considered for a package, recipients require an assessment by the Aged Care Assessment Team (ACAT).

Extended Aged Care at Home (EACH)


Funded by the Australian government to provide for the complex care needs of older people, Extended Aged Care at Home (EACH) packages are individually planned and co-ordinated packages of care, tailored to help frail older Australians to remain living at home.


Generally a person who is assessed as requiring high-level care (nursing home) could be eligible for an EACH

The types of services that may be provided as part of EACH include: registered nursing care, an allied health professional such as a physiotherapist, podiatrist; personal care, transport to appointments, social support, home help, and assistance with oxygen and/or enteral feeding. To be considered for a package, recipients require an assessment by the Aged Care Assessment Team (ACAT) - see listings page 41 for contact phone numbers for your area. An EACH package for people with dementia (called EACH-D) is also available.


Transition Care Program


Aims to help people to improve their independence and confidence after a hospital stay. Eligibility: While you are still in hospital, you must be assessed by an Aged Care Assessment Team (ACAT). This program provides a package of services including therapy, and personal and nursing care as part of an ongoing but slower recovery process. This means that you and your family or carer have time to consider your long-term care arrangements. Transition care is provided in your own home or in a ‘live-in’ setting, and is provided for a period of up to 12 weeks, with a possibility to extend to 18 weeks.


Community Nursing


General nursing care, wound care, medications such as insulin and oncology, support to clients and their families at home, advice on safety and mobility to assist recovery, to enable people to maintain their independence.
