SUPPORT


Home & Community Care (HACC) Program


Community care services to support people who are frail aged and younger people with disabilities, and their carers, to be more independent at home and in the community. Supports and assist people, who want to, stay in their homes for as long as they are can. Services funded through the HACC Program include: nursing care, allied health care, meals and other food services, domestic assistance, personal care, home modification and maintenance, transport, respite care, counselling, support, information and advocacy, and assessment.


Aged Care Assessment Team (ACAT)


Comprehensive assessment, information and support for the home-care needs of frail aged people living at home, including assessment for aged residential care. ACAT assists people to make informed choices about aged care services and to select those services most suited to their needs.
