| OPHTHALMOLOGY | ARTICLE The procedure was completed via a dissection of
the periosteal muscle and the creation of a flap attached to the orbicularis orbital periosteum with two or three Monocryl sutures.
Fat graft The author used a rigorous technique for the process of fat grafting, as part of an operating room with strict aseptic conditions. For the periorbital region, very fine cannulae are required. The amounts of fat to be injected varied. Generally these were: ■ Inner orbit: 0.5Ð 1 cc ■ Lateral orbital: 1Ð 2 cc ■ Malar region: 10Ð 15 cc ■ Upper eyelid: 2Ð4 cc (always submuscular).
Results The majority of patients were satisfied with the results of the blepharoplasty surgery; there was an improvement in the opening of the eyes and an overall rejuvenating effect. To evaluate this, the author sent a questionnaire to the patients and assessed comparative before and after photos (Figures 1–5). Overall patient responses were as follows: ■ Very satisfied: n=15 ■ Satisfied: n=20 ■ Dissatisfied: n= 1 ■ Very dissatified: n=0 ■ No response: n=9. The results of the RF sessions were variable among
patients, with a limited effectiveness in those with thicker skin. There was a reduction of fine lines and an improvement in the texture of the skin for those patients with thinner skin (mostly women). Phototherapy results with regard to solar elastosis
were all positive and improved the quality of the skin, and decreased spots and spider veins.

An improvement in the tear trough and a softer look to
the periorbital area was observed in patients who received a fat graft.
Complications In one case the upper eyelid scar was fragmented, so the suture had to be removed and then redone. There were two cases of superficial burns following RF to the lower
