Barntons Pharmacy has been selected as a finalist for their seamless collaboration with local surgeries, palliative care networks and training bodies. By prioritising palliative prescriptions alongside urgent medications, they ensure patients receive timely and compassionate care. Their Pharmacy First Plus initiative has eased GP pressures, offering rapid assessment and treatment for minor ailments. With staff training at its core, their efforts have helped future pharmacists and improved health literacy. From securing urgent medication for visiting patients to supporting families through end-of-life care, the team demonstrate how true partnership results in integrated, patient-centered care and community wellbeing.


KATHRYN HARVEY AND GP PRACTICES EDINBURGH


Kathryn Harvey led a city-wide initiative to optimise acute prescribing processes across 70 GP practices in Edinburgh. Working closely with Healthcare Improvement Scotland, she helped reduce the burden of acute prescriptions on GP surgeries, improving patient safety and access to repeat medications. The project achieved a 73% reduction in acute prescriptions in targeted BNF sections, including a 78% drop in acute antidepressant prescribing and a 90% reduction in combined oral contraceptives issued as acutes. Her work has freed up GP time, improved medication review processes and enhanced patient care through safer, more structured prescribing practices.


PRIMARY CARE PHARMACY WHINPARK MEDICAL PRACTICE, EDINBURGH


The Whinpark Medical Practice Pharmacy Team has excelled in multidisciplinary collaboration, organising interactive educational sessions for clinicians covering topics such as polypharmacy, anticholinergic burden and new treatment protocols. By strengthening links with smoking cessation teams, local community pharmacies and homeopathy practitioners, they have improved clinician awareness of local services and increased referrals to Pharmacy First Plus. Their efforts have led to more informed prescribing decisions, better medication reviews and a reduction in unnecessary prescriptions, ultimately improving patient outcomes and fostering stronger professional relationships between primary care and pharmacy teams.
