They have implemented structured polypharmacy reviews, resulting in reduced medication burden and hospital readmissions. By training pharmacy technicians in clinical observations and phlebotomy, they have enhanced patient monitoring and medication optimisation. Their efforts have resulted in 417 interventions over seven months, with projected annual savings of £72,000, improving both patient safety and healthcare efficiency. In recognition of their commitment, the team have been selected as finalists for this year’s award.
GOLDEN JUBILEE UNIVERSITY NATIONAL HOSPITAL PHARMACY TEAM CLYDEBANK, GLASGOW
The Golden Jubilee University National Hospital Pharmacy Team has revolutionised hospital pharmacy services by implementing "floor teams" embedded in wards, improving medicine reconciliation, patient engagement and discharge efficiency. This model has reduced discharge processing times by 44% and improved compliance in cardiology patients. Additionally, they have introduced pre-assessment pharmacy teams, optimising care for surgical patients. Their collaborative, patient-centred approach has strengthened multidisciplinary working, streamlined workflows and improved job satisfaction, making them a model for hospital pharmacy service transformation across Scotland and a worthy finalist for Hospital Pharmacy Team of the Year.
GASTROENTEROLOGY & RHEUMATOLOGY PHARMACY TEAM WESTERN GENERAL HOSPITAL, EDINBURGH
The Gastroenterology and Rheumatology Pharmacy Team at Western General Hospital has been selected as a finalist in recognition for their pharmacist-led initiatives, including biosimilar switches, post-induction review clinics and virtual biologic clinics, which have improved access to and efficiency of patient care. Their innovations have significantly reduced waiting times, improved treatment outcomes and generated substantial cost savings. They have also developed a patient database for tracking biologic therapies, strengthening clinical governance and research participation. Their leadership in service transformation has positioned them as national leaders in gastroenterology and rheumatology pharmacy services.
