Introduction to the heart Te heart is a rather special muscle, working


tirelessly to pump blood


around the body. Blood is brought to the heart by its three arteries, and problems of the arteries lead to chest pain, angina and heart attacks. Te heart’s control system is its electrical system, which generates the heart beat that can be seen on an ECG. An electrical impulse is generated by cells at the top of the heart, which then spreads via a network of cells through the heart. Electrical problems of the heart cause symptoms such as blackouts, palpitations, breath- lessness and, rarely, death.


When things are a bit slow Sometimes these electrical cells work slowly or stop working altogether. Tis leads to a slow heart beat, which is not always a sign of good health. One treats slow heart rhythms with a pace- maker, which is a battery powered device that generates an electrical impulse to stimulate the heart cells. It’s implanted under the skin and connects to the heart via leads placed in veins. Tere are different types of pacemaker, with one, two or three leads. A specific group of patients with heart muscle weakness benefit from a biventricular pacemaker with three leads to the heart. Tis pacemaker resynchronises the contraction of the main pumping chambers of the heart, and can not only improve symptoms but have a benefit on prognosis too.


When things go fast Fast heart rhythms arise from over- active cells or short circuits within


the heart. Most are benign, and are commonly called SVT. Some fast rhythms can cause sudden cardiac arrest. Treatment of fast heart rhythms is determined by the cause, and includes medications, defibril- lators and catheter ablation. Atrial fibrillation, where there’s electrical chaos in the upper chambers, is the commonest arrhythmia seen, and is more common in the elderly. Catheter ablation is performed from the vein in the leg and is more effective than medications in most patients. For SVT, ablation can be curative.
