Self-assessment sheets like the one below allow learners to give the teacher feedback on how they believe they performed on a specific task.

Formal assessment codes and percentages
A seven point scale is used for recording performance on formal assessment tasks. Teachers record the actual marks against the task by using a record sheet and report percentages against the subject on the learners’ report cards.
