SERVICE, BIGGART HOSPITAL Te Care at Home Pharmacy technician teams, an integral part of the Social Work and Primary Care Pharmacy teams, are pivotal in establishing appropriate pharmaceutical care for patients at home. Tey are located within Health and Social Care Partnership teams, having access to health and social care IT systems. Gathering relevant patient information is streamlined, providing essential links to ensure continuity of patient care.


DAVIDSONS CHEMIST, BRIDGE OF EARN Davidsons Chemists in Bridge of Earn serves a population of around 3,500-


4,000 patients with significant numbers of elderly patients and young families. When the local GP surgery closed last year, the pharmacy knew it had a massive task on its hands to transfer 3500 patients from BOE surgery to surgeries in Perth, Kinross and Newburgh surgery in NHS Fife. Te pharmacy team worked tirelessly and collaboratively with a number of healthcare partners from these areas to ensure that a high standard of care was continually being provided to the patients throughout this challenging process.


MELVILLE CHEMIST, RUTHERGLEN M&D Green provides one of the best medication services for care homes across


Scotland. Te company has been working in partnership with care homeowners, nursing staff, care and support staff and guidance from the care commission to enhance its medication services and, by working in partnership with many care homes, has transformed the medication service that it delivers. Te Director of the company has continuously developed the training package over the years, adapting PowerPoint presentations to account for changes in the way care homes deliver their medication services.


PRESCRIBING TEAM, SOUTH AYRSHIRE HSCP AND


ANTIMICROBIAL TEAM, NHS AYRSHIRE AND ARRAN Everyone is aware that there is a pressing public health crisis with regard to antimicrobial resistance. Te primary care pharmacy team in South Ayrshire HSCP has been developing significantly both in service provision and size for the past three years and now has a pharmacist and pharmacy technician in almost every GP practice in the locality. Te team supports the GPs by developing the pharmacotherapy service and supporting with all matters of prescribing. Tey influence the GPs throughout their daily work and aim to educate them in areas such as antimicrobial prescribing to inspire positive change.


LOCHARDIL PHARMACY, INVERNESS Lochardil Pharmacy is one of the busiest community pharmacies in Inverness and


works closely with the Highland Hospice: a purpose-built inpatient unit that aims to support patients in enabling them to live their best possible life and prepare for and experience the best possible death. Te pharmacy works closely with the hospice to support patients by supplying them with replacement medical supplies and devices and by providing discharge medicines to patients who are well enough to return home either to visit or to pass.
