EDINBURGH CANCER CENTRE PHARMACY
TEAM, NHS LOTHIAN PHARMACY Te Edinburgh Cancer Centre (ECC) Pharmacy team is led by a team of senior pharmacists and technicians and provides services from both the Edinburgh Cancer Centre (the specialist cancer hospital for the South East of Scotland), and St John’s Hospital, Livingston. Te team provides expert pharmaceutical care and first-class pharmacy services to patients living in Edinburgh and the Lothians, Dumfries and Galloway, the Borders and Fife. Te team strives to identify areas for improvement in managing demand and workforce development and ensures that optimum skill mix and collaboration are achieved. Te team is both are dynamic and innovative, and its innovations have paved the way for pharmacy practice, not only in Scotland but also across the UK.
PHARMACY TEAM, NEW CRAIGS HOSPITAL,
INVERNESS Te pharmacy team from New Craigs hospital is a very cohesive team with a common objective - to support the delivery of optimum pharmaceutical care to patients with mental health conditions across North Highland, regardless of whether they are in-patients or living in the community. Te team implemented a twelve-month pilot in two general practices in remote and rural Scotland, with patients with a diagnosis of depression and/or anxiety referred by general practitioners to specialist mental health pharmacist independent prescribers from the team for review and any necessary follow-up to ensure safe and effective use of medication. Whilst accepting study limitations, the initiative proved that there was potential to translate the pilot model of care to sustained services throughout general practice.
THE CARE AT HOME PHARMACY TECHNICIAN
SERVICE, EAST AND SOUTH AYSHIRE Te Care at Home Pharmacy (CAP) Technician Service in East and South Ayrshire is an excellent example of partnership working to deliver the best level of patient centred care across transitions. Te pharmacy technician works with elderly and/or vulnerable residents within the Health & Social Care Partnership (HSCP) area, who have been identified by a health or social care professional as requiring help with a medication related problem. Te CAP technician visits patients in their own homes to review concordance and compliance with medicines, with onward referral to other services where required. Te service initially focussed on elderly people discharged from hospital with a care package, but now receives referrals from the multidisciplinary team involved in adult care, including social workers and district nurses.
