End-of-Life Planning Conversations Important N
inety-nine percent of physicians say it’s important for health care providers to have conversations
with patients about end-of-life and advance care planning, and 95 percent support the new Medicare benefit that took effect Jan. 1. The survey from the John A. Hartford
Foundation, California Health Care Foun- dation and Cambia Foundation also iden- tified obstacles keeping physicians from engaging in these sensitive conversations including 46 percent of those surveyed say- ing they frequently or sometimes feel unsure of what to say. Less than a third said they had formal training specifically on talking with patients and their families about end- of-life care.
In January, the Centers for Medicare and Medicaid Services began reimbursing doc- tors and other professionals for these types of discussions. A 2015 poll from the Kaiser Family Foundation showed strong support for the benefit from the public and physi- cians, although only 14 percent of those physicians reported having billed Medicare for an advance planning conversation since reimbursement began early this year. A blog post from the Hartford Foun-
dation CEO Terry Fulmer notes that the survey also found that few health systems are set up to ensure consistency and regu- larity of these conversations. For example, one in four doctors said there is no place in their electronic health record indicating

if a patient has an advance care plan and for those who do, 54 percent say they can access the plan’s contents. The survey included 736 primary care
and specialist physicians in 50 states. The study can be found on the John A. Hartford Foundation’s website,
