Commissioning & Needs Assessment
Ambition 24direct works with the commissioning authority to review the objectives and procedures for the care programme. Our team then assesses the needs of each patient for care at home and in the community. The assessment process ensures that we can understand and evaluate the medical, care and personal needs of the patient - and choices of other Stakeholders, such as the family and registered carer.
The ‘Care Plan’ is a written document that is prepared following a face-to-face interview with the patient by a qualified nurse. The nurse is experienced in assessing each patient’s needs, professionally and sympathetically. The Care Plan confirms the requirements of the patient and how it is proposed that they are to be met.
Usually it is the patient requiring treatment who is interviewed for the Care Plan: if this is not possible due to age or infirmity, we can interview the nominated carer or family member. We may also ask for information from the GP, hospital consultant or other healthcare professionals involved. This is to ensure a clear understanding of the patient’s medical needs.
