Our complex care programmes are designed in close liaison with District Nurses, Community Health Care
Coordinators and Discharge Liaison Nurses
Gail is a two year-old girl. Her complex care needs relate to a tracheostomy and gastrostomy as well as cleft palate and micronathia. Gail receives night time support from the Ambition 24direct team on four nights a week, since 2010.
Our complex care programmes are designed in close liaison with District Nurses, Community Health Care Coordinators and Discharge Liaison Nurses, to provide care to patients within their own home. The skills of our nurses and care assistants in attendance must always match the needs of the patient and the specified programme.
We are able to provide staff who are experienced in ventilator care, including respiratory management and tracheostomy change, tracheal suction, ambubagging, chest percussion and administration of oxygen therapy. Skills can include gastrostomy and naso-gastric feeding as well as administration of medication, invasive bowel management and other care, where specified.
We work with health and social care professionals and services to ensure the most appropriate treatment and care for each Service User’s needs.
