EDINBURGH HEALTH AND SOCIAL CARE PARTNERSHIP, PRIMARY CARE PHARMACY TEAM, NHS LOTHIAN A collaborative effort between pharmacy teams and healthcare professionals to optimize stroke prevention in atrial fibrillation patients across Edinburgh. Education sessions, tailored patient talks, and data-driven interventions resulted in improved anticoagulation rates, benefiting over 1,300 patients.


CRIMOND PHARMACY, FRASERBURGH, ABERDEENSHIRE Crimond Pharmacy partnered with local healthcare teams to deliver PF+ services. Triage systems and additional training enhanced patient care, reduced GP workload, and improved health literacy. Streamlined processes led to prompt, appropriate care, benefiting 3,500 patients.


DAVIDSONS CHEMIST KIRRIEMUIR AND KIRRIEMUIR MEDICAL PRACTICE With close collaboration, this partnership improved access to healthcare services. Implementing Pharmacy First Plus (PF+), they reduced GP workload and enhanced patient satisfaction. The collaborative approach fostered trust, resulting in better health outcomes for the Kirriemuir community.


KINGSGATE MEDICAL PRACTICE PHARMACY TEAM AND HOSPITAL AT HOME TEAM The local GP practice and pharmacy established a Frailty clinic, addressing gaps in care for vulnerable patients. The initiative resulted in personalized care plans and reduced risks for patients. This partnership enhanced practice capacity and patient outcomes.
