WESTERN GENERAL HOSPITAL, NHS LOTHIAN The Acute and General Medicine Pharmacy Team exemplifies dedication to the pharmacy profession, offering proactive clinical services and fostering skill development among pharmacists, technicians, and support workers. Their initiatives, such as integrated ward rounds and streamlined discharge processes, have enhanced patient care and medication safety. Strategic planning and collaboration have increased the efficiency of patient reviews, supported patient flow and reduced out of hours medicines supply requests. Their interdisciplinary approach and strong relationships with medical consultants ensure ongoing service improvement and expansion.

NHS TAYSIDE MENTAL HEALTH PHARMACY TEAM A continual drive to improve patient care and a desire for more clinically rewarding activities has enabled the Mental Health Pharmacy Team in Tayside to demonstrate commitment to patient-centred care by expanding roles across in-patient and post-discharge settings, improving patient outcomes, and integrating pharmacy services with community mental health teams. Skill-mixing enhanced pharmacy input, with pharmacist-led initiatives like the Lithium clinic significantly improving patient monitoring. The multidisciplinary team streamlined processes and ensured continued service improvement with patient-focused initiatives which have strengthened pharmaceutical care delivery in the hospital setting.


XXX ANTIMICROBIAL AND SURGICAL TEAMS, UNIVERSITY


HOSPITAL WISHAW, NHS LANARKSHIRE The Antimicrobial team developed and updated policies, trained new staff, and launched a crucial Out-Patient Antibiotic Treatment (OPAT) service. This service allowed safe discharge of patients on IV antibiotics, improving patient flow and reducing strain on departments like orthopaedics. Additionally, they mentored junior pharmacists on research and quality improvement projects, leading to presentations at national conferences. Despite post-COVID recovery challenges, the team demonstrably improved patient care and reduced workload and environmental impact. These efforts, resulted in improved patient care, standardized practices, and better communication across the healthcare system.
