A prospective cohort evaluating fish consumption in 210 healthy men†
aged 70 to 85 years
demonstrated that fish consumption of more than 20 g/d (equivalent to 347 mg EPA plus DHA per day) resulted in less cognitive decline at 5-year follow-up.47
A 2016 dose-response
meta-analysis of 21 cohort studies demonstrated that both increased fish intake and a 100-mg/d incremental increase in DHA intake were associated with a 14% lower risk of dementia and a 37% lower risk of Alzheimer disease.48 A clinical trial assessing patients with mild to moderate Alzheimer disease showed that
supplementation with 2 g DHA for 18 months did not slow the rate of cognitive decline compared with placebo for 295 participants.49
However, a subgroup of patients with mild
cognitive impairment experienced a significant reduction in cognitive decline with fish oil supplementation (1,290 mg DHA and 450 mg EPA daily).50
Overall, the findings from RCTs
suggest that omega-3 supplementation with EPA or DHA does not affect cognitive function in older healthy individuals or people with Alzheimer disease. However, DHA supplemen- tation (≥1 g/d) may impart improvements in cognitive function. Further research on this topic is needed.
