The TSL supervisor carried out Daily Task Briefings based on the site specific risks for the particular day. In addition to the briefing, BFK produced a start of shift activity plan (SOSAP) every 7 days or when new work activities were planned and operators briefed accordingly, incorporating the BFK daily briefing, in order that TSL operatives understood the other activities being carried out within the shaft (with up to 40 other BFK or Subcontractor operatives working in the shaft). The shaft was defined as a “highly restricted space”.
Other documentation produced during the works included:

• Gas Monitoring Records (Oxygen, H2S, CO, methane)
no hold-ups on site. Because the works were carried out 30m deep in Crossrail shaft SHW2 no consultations were required with the public or nearby business. However each delivery to site (pipes, equipment etc.) required the vehicles to be electronically booked in using BFK‟s vehicle booking form at a particular time slot and all vehicles had to comply with Crossrail‟s rigid vehicle safety requirements for vans up to Heavy Goods Vehicles.
In advance of the works additional insurances were required for every TSL employee, to include further accident & life cover. In addition to this all TSL employees completed a pre-placement health declaration and submitted records of training. The standard Crossrail drug & alcohol tests were also carried out as well as site specific inductions before works could commence.
A detailed construction methodolog with associated Risk Assessments and Construction Phase Health and Safety Plan was produced by TSL to incorporate Crossrail/BFK‟s policies, format, sequencing, risk quantification, H&S and Environmental control measures, access measures, welfare requirements etc. As well, a precise inspection and test plan was produced that included, inter alia, activities such as loading of the steel thrustframe, setting out of thrust bore pipe, accuracy and approvals for breakthrough into the SCL tunnel, accuracy and recording of the Terminator position within the tolerances given etc.
When all approvals were acquired, programme agreed, and plant & labour mobilised, the first task was to cut out a 500mm x 600mm section of the 1.2 metre secant piles i.e. the walls of the SHW2 shaft.
The TSL construction team were briefed on the contents of the detailed method statement, with each member of the site team signing the document after reading.

After temporary works designs were submitted and approved, the 500mm x 600mm x 1200mm (thick) secant pile was excavated using air tools. This dimension was the maximum crossrail would allow, and was just enough to allow the 480mm outside diameter of the Terminator Head (including over-cut). The position of the hole had to be exactly perfect, leaving none too little margin for setting out. As such the positioning of the excavation was surveyed and checked.
After excavation the void was filled with Hilti CP637 structural grade firestop mortar to BS476 standard. This grout was chosen due to its 24hr curing time and weak compressive strength after full cure i.e. 12.3 N/mm2.
After curing a concrete headwall was formed and works began on the independent backwall or thrustwall restraint. Before works commenced on the
