Moderate Malnutrition in the Context of Acute Disease or Injury Moderate malnutrition related to inadequate oral intake in the setting of acute disease or injury (acute pancreatitis) as evidenced by <75% of EER for >7 days (1-2 small meals or snacks/day vs. usual 3 meals or snacks/day); significant weight loss of 1% (2 lb) over the last week; mild subcutaneous fat loss (orbital fat pads, buccal fat pads) and mild muscle wasting (temporalis, pectoralis, deltoids).


Moderate Malnutrition in the Context of Chronic Disease or Condition Moderate malnutrition related to increased protein needs in the setting of chronic disease or condition (ESKD on HD with losses of protein via dialysis) as evidenced by <75% of estimated protein needs >7 days (decrease in protein intake for 4 weeks); mild muscle wasting (temporalis, pectora lis, deltoids, trapezius) and mild subcutaneous fat loss (orbital fat pads, triceps, and ribs); decrease in functional status (now assis- tance needed with ADLs).


Moderate Malnutrition in the Context of Starvation Moderate malnutrition related to inadequate oral intake in the setting of starvation (due to limited access to food in setting of home lessness) as evidenced by consuming <60% of estimated energy needs per diet history for greater than 4 months; mild subcutaneous fat loss (triceps, ribs) and mild muscle loss (pecto- ralis, deltoid, trape zius, supraspinatus, infraspinatus, quadriceps, gastrocnemius).
