MEMBERSHIP INFORMATION 1. Members are required to present their membership cards when using the facility. $15.00 fee for replacement cards.


2. All participants must check in at the front desk to scan their card.


3. Please leave your valuables at home or locked in your trunk. We can not be responsible for lost items.


ABEYANCE POLICY The holding of membership in abeyance for vacation, short term illness, business trips, etc. is done on individual basis. • There will be a $20.00 service charge for any returned check.


• There will be no credits/refunds allowed for days missed due to personal/health reasons.


LOST AND FOUND


Lost and found items are held in the Front Desk area. Articles are given to worthy community agencies if they remain unclaimed after 2 weeks of storage.

CODE OF CONDUCT


All participants should recognize that the Peninsula Family YMCA is dedicated to the benefit and welfare of all its participants. In order to keep a consistent and cooperative atmosphere, the Board of Management asks that all participants at the facility follow these guidelines: 1. Refrain from the use of foul language, abusive actions, and inappropriate behavior.


2. Recognize that respectful conduct, fair play, and a cooperative attitude govern behavior at all times. 4. No smoking, alcohol or drugs 5. No cell phones in wellness center or locker rooms.


6. No boys over the age of 4 in the Women’s lockeroom; No girls over the age of 4 in the Men’s lockeroom.


7. No pets allowed unless they are “working dogs.”


8. No loitering outside premises. 3. Failure to observe this code will result in suspension or terminations of privileges.

There are two choices for membership payment: • ANNUAL or BANK DRAFT (ATS):


ANNUAL MEMBERSHIP — You may pay for your membership in full for the year by check, cash, or Visa/Discover/ Mastercard. There is no difference in the annual fee if you pay in full or by bank draft (ATS). No refunds. Should your membership lapse over 30 days you will be required to pay Joiners Fee upon renewing.


BANK DRAFT MEMBERSHIP (ATS)— Our bank draft membership is designed to give our members a more convenient way of paying for their membership. With your permission, an automatic withdrawal will be made against your bank account each month.


NOTE: If you need to cancel your member- ship, a 15-day written notice is necessary.


REFUND POLICIES


The entire fee for programs is due at the time of registration. All refunds and transfers for classes must be requested five working days prior to the first class meeting. When a refund or transfer is requested, there will be a $5.00 service charge. No refunds thereafter.


• Camp deposits are nonrefundable.


• Refunds or credits may be given when any class/program is cancelled by the YMCA.


• Y Memberships are NOT REFUNDABLE under any circumstances.
