he Chronic Medication Service (CMS) has become an integral part of the day-to-day working within community
pharmacy. With that in mind, Andrew Watson and his team at Davidsons Chemist in Bridge of Earn identified the need to develop a focus on quality pharmaceutical care activity, and, more importantly, excellence in pharmacist intervention for patients with long-term conditions. Andrew and his team were determined to go that ‘extra mile’ as part of their CMS

intervention, looking at the patient holistically and not just focusing on their long-term medical condition.
The examples of Andrew’s success in this initiative are varied and numerous..
First of all, Andrew identified that, during all CMS reviews, the most common complaint highlighted from patients was the fact that ‘all their tablets ran out at different times’, and so he began to think about the synchronisation of patients’
medication on a wider scale. Andrew determined that, if he could synchronise patients’ medication before they received their first serial prescription, this would - in turn - improve patients’ compliance/ concordance.
Patients in Bridge of Earn were then encouraged to either bring in a list of the exact number of tablets they had left, or to bring in all their medication to their CMS review so that Andrew could count it (and check if it was
still needed/in date) and then use this information to synchronise the medication by writing CPUSs and providing the medication required.
One patient with polypharmacy was also attending the pharmacy every week to pick up various items associated with his hypertension and type 2 diabetes. He would very frequently run out of testing strips, needles, medication and even his insulin. The patient expressed his
