ith such statistics in mind, the latest polypharmacy pilot involving community


pharmacists conducting medication reviews for patients within care homes that's being tested by NHS Greater Glasgow & Clyde (NHSGGC) is to be welcomed. Pharmacist, Elizabeth Roddick, from New life Pharmacy on Clarkston Road in Glasgow is one of the pharmacists taking part in the pilot.


'There's no doubt that polypharmacy in care homes is of particular interest to GPs and pharmacists,' Elizabeth told Scottish Pharmacist. 'Care homes can house up to 100 or so residents,


and it's highly likely that most of these elderly and infirm residents are on medication in some form and often have increased pharmaceutical care needs. Up until recently, these patients' medications might not have been regularly monitored, and healthcare professionals could be just involved in acute call-outs where a patient may have fallen, or was suffering from a UTI. What was needed was a new approach, using the skills of the community pharmacist, which would look at patients' medicines to not only ensure compliance and adherence, but to look at the overall picture to check which medicines they were taking and why.
