s with most European countries, Scotland is experiencing the growth of an ageing population, so care homes are going to becoming increasingly prominent in the community over the next few years. Naturally, pharmaceutical care in these care homes is of paramount importance, so it’s great to see the vital work being undertaken by the Care Homes Pharmacy service in NHS Lanarkshire, which won this year’s Innovation & Change in Pharmacy Practice Award.

People living in care homes are generally prescribed a variety of medicines and have a number of co-morbidities. By visiting both the care home and the GP practice, and pro-actively reviewing the full medicines regime for these residents, a pharmacist is able to ensure that all medicines prescribed for the patients remain appropriate and, by involving their carers and family members in the decision-making process, the pharmacist can significantly reduce the number of medicines


that the residents in these care homes are taking, thereby ensuring a safe, effective and person-centred medicines regime.


This year, with the agreement of each patient’s GP and family members, two of the NHS Lanarkshire Care Homes Pharmacy Service team’s community pharmacists, Paul Pollock and Ruth Campbell, took on long- term prescribing responsibilities for medication review in care homes that don’t benefit from the NHS


Lanarkshire GP local enhanced service and where, as a result, residents don’t have regular medication reviews. In line with the vision of Prescription for Excellence, Paul and Ruth were each allocated two care homes with which they both work and support on a long-term basis. The pair began to carry out holistic reviews for every resident in these homes and prescribed the changes at each patient’s GP practice across a wide range of clinical areas. This long- term role within a care home setting
