CLINICAL TEAM GAIN ACCLAIM AT UHA


The Pharmacy Department within the hospital have received recognition for their innovation and teamwork, awarded the Hospital Pharmacy Team of the Year at the Scottish Pharmacist Awards.

Senior Pharmacist within Surgical Services and Critical Care, Sarah Brown tells us, “The hospital has an accident and emergency department and the beds are a mixture of medical and surgical including orthopaedic.

University Hospital Ayr (UHA) is an acute NHS hospital serving the south of NHS Ayrshire and Arran.


Traditionally, the clinical service allocated pharmacists to specific wards, however under this system the team were unable to cover all wards at all times.


“This led to some patients never being reviewed by a pharmacist during their hospital stay. It was recognised that this caused inequalities in the service provided to patients, as well as introducing a level of risk where a pharmacist was not involved in checking patients’ prescribed medication.


“It is well recognised that most


medication errors take place on admission. The EQUIP study found that 13.4% of prescriptions written on admission contained errors while the PROTECT study quotes that out of all the errors identified by the authors 56.7% were made on admission. As such, since September 2013, the clinical pharmacy team has completely changed the way that the clinical pharmacy service is delivered, to ensure that the risk associated with medication errors are reduced, and to realise a more person-centred service, in line with Prescription for Excellence.”


To this end, a number of changes have been made to allow the team to provide the best service possible. UHA benefits from a Hospital Electronic Prescribing and Medicines Administration (HEPMA) system, and the team’s first move was to develop a report using information available from this system.


This report runs automatically twice daily and identifies all the patients within the hospital who have yet to be reviewed by the clinical pharmacy team. With this information, the clinical pharmacy team meets first
