and other tests, request prescription renewals, receive automatic reminders about important health screenings and access authenticated, personalized health information that is truly relevant to their needs. Connectivity, security, safety, and a coordinated medical group

practice model through which physicians deliver high quality healthcare to patients who are empowered to knowledgeably participate in all aspects of the healthcare services they require - that is what Cleveland Clinic’s integrated electronic medical record system was designed to support. Increasingly, this same system is being recognized as a model of healthcare efficiency that, adopted by more of our nation’s physicians and hospitals, could potentially address issues of healthcare quality and cost on a much broader scale. For example, physicians who use integrated health information technology tools do not have to repeat medical tests because the test results they need are always available. Costs related to such chronic illnesses as diabetes and hypertension can be better controlled when patients who have access to secure monitoring tools can collaborate with their physicians and more effectively control their own conditions. Such technology-enabled patient/physician connectivity can result in fewer condition-related complications, fewer emergency room visits, and ultimately fewer hospital admissions. Cleveland Clinic’s multidisciplinary group practice model has, as its central purpose, the delivery of healthcare of the highest possible quality to every patient. Cleveland Clinic’s electronic medical record system is a 21st

century reflection of that same, patients first

commitment, and its secure, technology-based tools are used by Cleveland Clinic physicians and associated caregivers to positively impact the lives of patients everywhere. +

Available to physicians in private practice, the MyPractice community electronic medical record service is a specially designed version of the same EMR system used by Cleveland Clinic physicians. It is a flexible and affordable system used by many area physicians to manage their busy private practices, and it also helps coordinate the care of patients if they require specialty services at Cleveland Clinic. For patients, MyChart’s personal health record service securely connects patients to portions of their own, physician-entered electronic medical record. Since 2004, over 350,000 personal MyChart accounts have been requested by Cleveland Clinic patients, who use MyChart to directly schedule appointments, receive the results of laboratory
