Aspecial “passport” that aims to help people living with de-

mentia feel more comfort able in their surroundings while away from home has been launched. The launch of the “This Is

Me” dementia passport con tains a photo of the person along with their likes, dis likes, preferences and daily routines to ensure the tran sition from living at home to a healthcare setting is as

smooth as possible. Theschemewasoriginally

developed by the Royal Col ,• rj~ i.V,

lege of Nursing in partner ship with the Alzheimer’s Society and it is now being re-introduced by the Older Mental Health and Learning Disabilities Service at Lanca shire Care NHS Foundation Trust. The passport is being rolled out in dementia wards pro vided by theTrust and sup ports health and social care staff in gathering personal and life history informa tion from individuals with the disease enabling them to deliver care that is specifical

ly tailored to the individual’s

needs. It is designed to be completed bytheindividualwherepossi- ble, family members or their carer. The passport can help carers gain a better under standing of the person living with dementia arid help re duce some of the difficulties they may experience around communication. It can also assist in ensuring their likes and dislikes regarding such things as food and drink are conveyed to the people who care for them.
