dition of the gastro-intestinal tract, caused by gluten, a protein in wheat, and similar proteins in rye, barley and oats. .


Gluten damages the lining of the small intestine


which greatly reduces the ability of the gut to absorb adequate nutrients from food and can lead to severe illness resembling malnutrition. In the 1950s it was thought to be almost exclu­


sively a childhood condition, but-now it is recog­ nized th a t many more adults than children are diagnosed. Coeliac symptoms can manifest themselves at


any age and, according to Coeliac Society statis­ tics, most coeliacs are diagnosed when aged between 30 and 45 years. Recent figures even show


more over-60s than under-16s being diagnosed in a year. Coeliac patients may be acutely and severely iU


with weight loss, vomiting and diarrhoea or they may have chronic almost trivial symptoms such as tiredness, lethargy and breathlessness. Usual S3rmptoms would be somewhere between


these extremes. Adults may have a history of abdominal or intestinal upsets, or they may sud­ denly develop the condition at any time. Anaemia, abdominal discomfort, mouth ulcers


and weight loss are common features. They should not treat themselves with a gluten-free diet before seeking medical help. Some people are diagnosed as coeliac without having obvious symptoms. For example, the condition may be picked up when rel­ atives of a coeliac are being studied. • The only treatment necessary to return the


intestine to normal is a strict gluten-free diet. Sometimes vitamin or mineral supplements may be


required to start with. The hospital consultant will know from tests what the patient needs, and sup­ plements should not be taken without' medical supervision. To avoid serious complications of the condition, a strict gluten-free diet is necessary for life.' Coeliac disease used to be rare, but more and


more doctors are beginning to recognize the disor­ der in their patients. The average incidence in the United Kingdom is 1 in 100 people. It is also known that the condition runs in families. Here is a recipe for a main course of beef with

• lkg/21b rolled Sirloin • salt and black pepper • 1/2 teasp dried thyme • 3 oranges u • 3 tbsp oil • 150ml/5 fl.oz. hot fresh beef stock


' • 2 tbsp cornflour • 150ml/5 fl.oz. single cream • 1 teasp grated orange rind • 1 tbsp Grand Marnier • 1 tbsp Cognac


Method


: 1. Preheat the oven to 220C, 425F, Gas mark 7. 2. Score the fat on the meat with a sharp knife and season the meat all over with salt, thyme and pep­ per then pour over the orange juice from two oranges, catching the drips in a dish. 3. Heat the oil in a roasting tin, add the beef and brOvm on all sides. Remove from the heat and add the orange juice. Roast in the oven for 1 to 11/2 hours according to taste, basting occasionally. 4. At the end of the cooking time, remove the meat


■ from the oven and place on a serving platter sur­ rounded by thin slices of the remaining orange. Keep warm.


' 5. Meanwhile, make the sauce by adding the stock to the cooking juices in the roasting tin. Cook for a few minutes then cool slightly and skim off any fat. 6. Mix the cornflour with a little water then stir in half the cream. Add to the roasting tin containing the stock and cook over a gentle heat, whisking all the time until well mixed then gradually add the remaining cream. 7. Sprinkle in the grated rind, salt and pepper. Stir well and cook gently for a couple of minutes. 8. To serve - warm the Grand Marnier and Cognac in a small pan or ladle, pour over the beef and carefully set alight.
