Sometimes the biggest safety lessons come from the smallest objects.
An EC135 helicopter sustained substantial damage after an unsecured fire extinguisher cover was blown into the fenestron during landing. The pilot safely landed the aircraft, and no one was injured.
The NTSB determined the accident was caused by the improperly secured cover, reinforcing the importance of FOD awareness around every landing zone.
