(PACT) PACT is using the unique network of 525 community pharmacies to deliver programmes in partnership with other healthcare providers, local community and voluntary sector organisations and host communities. PACT has developed a new ‘model of care’ for the network, which includes a community-led medicines support project, the IMPACTAgewell Project, an atrial fibrillation project and Project ECHO.


BANNSIDE PHARMACY


PORTGLENONE The Portglenone Diabetes Partnership initiative involved collaboration between community pharmacy, Northern Health & Social Care Trust clinical psychology, Mid & East Antrim Council and Portglenone Enterprise Group. As a result of this collaboration, a group of patients with Type 2 diabetes on a physical and psychological basis to an excellent range of outcomes.


STEPHANIE HAUGHEY


MCCARTAN’S PHARMACY, NEWRY Locum pharmacist, Stephanie Haughey, worked in conjunction with the National Childbirth Trust to support pregnant women and new mothers in a range of issues, from breastfeeding support to self-care. Stephanie called on a wide range of resources and bodies with whom she worked in partnership, and is now designing a new cross- generational project, which will involve grandparents and children.


CROSSINS CHEMIST


BELFAST Pharmacist Kevin McDevitt worked in conjunction with Action Mental health to deliver a project for local women centred on personal development. Kevin focused on a range of subjects such as ‘what is mental health?, drugs and how they work, and a variety of sessions on physical health, such as diabetes, obesity and heart disease. He is now involved in designing a project around health inequalities.
