Encouraging Independent Living This year we expanded our programs for adults with disabilities who want to live as independently as possible while pursuing meaningful personal goals. In 2010 we:


• Inaugurated the Yellow House in Norwood, allowing young adults with disabilities to live independently within a supportive community.


• Opened 10 new independent living apartments in Brookline.


• Began a case management program for adults with psychiatric disabilities.


Aging with Dignity We are proud of the many programs and services we offer to enhance seniors’ quality of life, encourage independence, and provide a safe and supportive living environment.


• Your Elder Experts is now the largest care management practice in Greater Boston, offering one-time consultation, crisis management, and ongoing, day-to-day care management for elders, their families, and caregivers.


Welcoming Immigrants We served 60 people seeking citizenship from Angola, Portugal, the Dominican Republic, Somlia, Uganda, Haiti, Russia, Latvia, Moldova, Ukraine, Georgia, and Burma with a one-year grant of over $99,000.


Providing Home Care Our Visiting Nurse Association, GAFC, and Home Care have continued to grow beyond their core areas of Boston, Brookline, and Newton, resulting in new clients in Brockton, Canton, Dedham, Norwood, Sharon, and Stoughton.


• Formed community collaborations with organizations such as the Norwood and Brockton Housing Authorities.


• Launched a pilot program to examine avoidable hospital readmissions and care transitions, and partnered with Beth Israel Deaconess Medical Center on the State Action on Avoidable Rehospitalizations (STAAR Initiative).


It’s hard to ask for help. I’ve always worked, and I used to be a donor, but now without Family Table, I literally could not feed my son. Whether someone gives $5 or $5,000 – I hope they know they are making a difference.
