How can we help?
We can assess your situation to find out if you are entitled to social care support from us. Don’t worry if you are not entitled to support. We can still give you information and advice and suggest other groups and services which can help.
Assess: This is where we help you think about what you can do for yourself, what you need support with and what sort of support would be best for you.
What will happen next?
We will look at how you can be as independent as possible, by using our enablement services. You might use our intermediate care services, telecare and mobility equipment to see if these help.
Enablement: Mobility aids and rehabilitation to help you get you as fit, well and independent as possible.
Intermediate care: Services that you use for a few weeks. They help you to get your confidence back and help you to do everyday tasks. The services are good for people who have been in hospital or who have been unwell.
Telecare: Equipment such as sensors which know when there is a problem and automatically alert either Care on Call mobile warden service or your carer.
