issue sagging is one of The mosT prominenT feaTures of The ageing face. Traditional facial rejuvenation treatments focused on soft tissue descent, and the mainstay of facial rejuvenation was the surgical facelift. During the last decade, however, great advances have been made in our understanding
of ageing and it has been established that laxity is not the only factor that leads to tissue sagging. We now know that both our soft tissue and skeleton change tremendously and
that deflation, together with laxity, lead to tissue descent by gravitation. rapid technological advances have characterised the new millennium by allowing us to treat facial
deflation with volumising fillers, and to treat tissue laxity using tightening devices. at the moment, volumising hyaluronic acid and other soft tissue fillers are the mainstay of facial
volume loss treatment. The different fillers, with their different rheological characteristics and their very high safety profile, enable us to volumise and restructure the face. We can volumise the deep plane by injecting above the bone, thus addressing morphological bone changes; loss of volume in the fat compartments of the face can be treated by injecting into the subcutaneous fat; and the more superficial volume losses can be treated using superficial injections into the dermis. The correction of volume in the right plane will lift the soft tissue, giving us the ‘injectable’ or ‘filler’ face lift. further lifting of tissue can be achieved by tissue tightening. multiple devices that heat the skin and
lead to collagen remodelling — and possibly new elastin production — are now available. many devices that are able to induce tissue tightening use radiofrequency technology (monopolar or bipolar) for tissue heating, while other devices use light sources to heat the skin. even though remarkable results can be achieved with some radiofrequency devices, many times the amount of tightening is inconsistent and results may vary. We are now beginning to understand which is the right treatment protocol for which indication. further studies on and clinical experience using radiofrequency devices may enable us to achieve both consistent and remarkable clinical results in the future. our patients’ demand for non-invasive procedures can be demonstrated
by their growing popularity. according to statistics from the american society for aesthetic plastic surgery (asaps), botulinum toxin injections increased by 50% from 2002 to 2010, and hyaluronic acid injections are now second in popularity to botulinum toxin. Combining fillers to volumise and a tissue tightening device for
further lifting offer us and our patients a non-invasive treatment possibility for tissue sagging, with remarkable improvement and nearly no downtime, and very high patient satisfaction as a result.
