ianne Urban moved to Landis Homes four years ago. With the assistance of Residency Planning Coordinator, Deb Kraybill, and the Facilities team, Dianne came to enjoy the outside accessibility


and patio in her apartment, in addition to the updated countertops, flooring, and appliances. However, she wondered how all her things would fit in the new space. Te Landis Homes Residency Planning team called on Kim Dyer and Rick Bernhardt of Graceful Transitions, a service of Landis at Home, to help Dianne.


“Graceful Transitions works with clients in all aspects of the move process,” says Kim Dyer, Coordinator.


Once Kim and Rick had finished directing the move of Dianne’s items to her new apartment, the real work began – laying out the apartment to make it truly feel like ‘home’. “When Kim and Rick showed up, they provided me with all of the help I needed,” Dianne explained happily. “Tey assisted with packing and unpacking, donating and getting rid of old and unnecessary items, and determining where the furniture would go.” Kim also measured each furniture piece and physically mapped out her apartment, so Dianne was able to visualize the layout before moving in. “I was shocked to see how much room I had in my new place!” Dianne exclaimed. “Tey could not have done a better job in arranging the furniture. Te place looked great!” Once all of the furniture and décor was moved in, Rick even assisted Dianne with hanging all of her pictures.


“What I love most about working with Graceful Transitions and clients like Dianne Urban is the relationship that we build through the process of downsizing, moving and transitioning,” says Kim. “From the initial consultation and periodic check-ins over the course of a move process to doing the actual pack, move and unpack, we learn to know our clients well as we take time to listen and be present with them. We learn about their families, their story, their likes and dislikes, what means most to them, etc.” Working with individuals and their most personal possessions opens up doorways for connection, and this creates a meaningful relationship with clients and families.

Dianne exclaimed, “Rick and Kim are great! I thank them both so much, and everyone that helped me and supported me through this move. I couldn’t have done it without them!”
