An example of a patient’s journey at 90 Sloane Street A fi t and alert 90-year-old man, well- known to his doctor, came in: “I’ve been panting when I walk for


three days,” he said. “Are you short of breath and do you


have a cough?”, the doctor replied. “No just panting, that’s all Doc.” T e patient’s oxygen satura-


tion meter reading was low at 95%. “Could this be a pulmonary embo- lism?”, thought the doctor, as he listened again to the perfectly clear air entry to the patient’s chest.


“Or could it be cardiac?” On-the-spot blood tests were


taken including D-Dimer for clot- ting and Troponin for heart attacks. T e CRP for infection was normal. T e patient went straight upstairs for an echocardiogram and an ECG. T e experienced echo technician


rang 15 minutes later: “T e right heart is dilated with


raised pulmonary pressures, and the left ventricle is normal,” she said. (T ese are typical features of a pulmonary embolism.) T e D-dimer blood test was posi-


tive for clotting, making pulmonary embolism likely. T e patient was given oxygen and heparin treatment


in the practice


before going to hospital for a more formal diagnosis. Ten days later the patient came


back smiling: “T e panting has stopped Doc.” (Case changed for confi dentiality). Many pulmonary embolisms are
