The path to receiving a Home Care Package can seem confusing. Let us make it easier to navigate so you can start receiving services sooner. Follow these simple steps and you’ll be well on your way to receiving the help you want and need.

ARRANGE YOUR AGED CARE ASSESSMENT Contact My Aged Care on 1800 200 422


Staff will provide information and ask some questions to see if you are eligible for a Home Care Package. If you are, they will arrange a free assessment with the Aged Care Assessment Team.


COMPLETE YOUR ASSESSMENT You will be visited by the My Aged Care Team who will assess your needs and discuss the process with you.


RECEIVE YOUR OUTCOME You will be sent a letter to let you know if you are eligible. If you are, the letter will also advise you of the level of Home Care Package you can receive. You will then be added to the national Home Care Package queue.


BE ASSIGNED YOUR HOME CARE PACKAGE You will receive a letter from My Aged Care when you have been assigned your Home Care Package, which is based on your position in the national queue. You will be given a unique referral code to give to your chosen provider.
