Mae protocol personcoll newydd ar gyfer pobl sy’nbyw gyda Dementia wedi cael ei gyflwyno ar draws ardal Heddlu Dyfed‐ Powys. Cynlluncenedlaethol yw Protocol Herbert a gyflwynwyd ganyr


heddlu mewnpartneriaeth gydag asiantaethau eraill sy’nannog gofalwyr i gasglu gwybodaeth ddefnyddiol a allai gael ei defnyddio petai rywunbregu s ynmynd ar goll. Mae’r Protocol yngofyni ofalwyr, teu lu a ffrindiau gwblhau

rhyfel fu ’n rhan o Lanfeydd Normandi, a oedd yn byw gyda dementia. Bu farw George Herbert wrth chwilio am ei gartref pan oedd ynblentyn. Beth yw Protocol Herbert? Dogfensyml i’w ddefnyddio petai rywunsy’nbyw â dementia yn mynd ar goll. Ffu rflenydyw sy’ncynnwys manylionpwysig person bregu s sy’ngallu cael ei rannu gyda’r heddlu os bydd y personynmynd ar goll. Dylid cadw llun diweddar o’r persongyda’r ffu rflen. Nid yw hynynmynd i gymryd lle unrhyw fesu rau diogelwch cyfredol. Pwy sy’n cwblhau’r ffurflen? Mewnsefyllfa o ofal, gall y gofalwr, y personbregu s neu ’r teu lu lenwi’r ffu rflen. Dylid cwblhau a diweddaru ’r ffu rflenynrheolaidd, fel bod yr holl wybodaeth mor berthnasol â phosibl. Pryd ddylid anfon y ffurflen at yr heddlu? Dim ond amser bydd personynmynd ar goll fydd yr heddlu angeny ffu rflen. Nid oes angeni’r heddlu gael y ffu rflencynhynny a dylid dychwelyd y ffu rflenunwaith y bydd y personwedi’i ganfod. Ble ddylai’r ffurflen gael ei storio? Dylid ei storio’nddiogel yny mano ofal ond mewnlle y gellir ei ganfod yngyflym. Adref, dylai’r teu lu fod yngallu edrych ar y ffu rflenos yw’r prif ofalwyr ddim yno ar y pryd.
